Ekonomická analýza účinku chemoprevence u žen se zvýšenýmrizikem vzniku karcinomu prsu

Abstract

Diplomová práce je zaměřena na problematiku chemoprevence karcinomu prsu. První část práce je věnována primární a sekundární prevenci a mamografickému screeningu. Další velmi důležitá a podstatná část práce je věnována právě chemoprevenci. Jsou zde uvedeny látky, u kterých byla testována chemopreventivní účinnost, výsledky klinických studií s těmito látkami a jaké nežádoucí účinky s sebou přineslo užívání těchto látek. V další části jsou zmíněny farmakoekonomické nákladové analýzy. Hlavní část diplomové práce se zabývá ekonomickou analýzou účinků chemoprevence. Tato analýza byla provedena prochemoprevenci tamoxifenem. Započítání nákladů na léčbu nežádoucích účinků tamoxifenem se ukázalo jako statisticky bezvýznamné, tudíž náklady na chemoprevenci činily pouze náklady na preparát. V poslední části práce byla určena prahová hodnota pětiletého rizika vniku karcinomu prsu, od které je užívání chemoprevence nákladově efektivní. Tato hodnota je 2,7 %

The thesis is focused on the problematic of breast cancer chemoprevention. The first part of the thesis is devoted to primary and secondary prevention and to mammographic screening. The other very important part of the thesis is devoted to chemoprevention. In this part the various chemicals which have been tested for their chemo-preventive features are mentioned aside with their side effects and clinical studies results. Next part discusses the pharmacoeconomic analysis. The main part of the thesis deals with economic analysis of the chemoprevention effects. This analysis was carried out for the chemoprevention by tamoxiphen. Inclusion of the side-effect treatment expenses showed up as statistically unimportant, meaning that the expenses for chemoprevention corresponded with the expenses for the medication. In the last part of the thesis the threshold value, stating the risk of development of the breast cancer from which is the usage of the chemopreventive medicaments cost-effective, was determined. The value is equal to 2,7 %
