Klinicko-ekonomické analýzy zdravotnických technologií v České republice

Abstract

česky: Klinicko-ekonomické analýzy zdravotnických technologií v České republice: Klinicko-ekonomické analýzy zdravotních technologií slouží v systému HTA pro podporu objektivního rozhodování autorit zdravotního sektoru. Cílem této diplomové práce bylo provést systematickou rešerši klinickou-ekonomických analýz zdravotnických technologií vydaných v posledních deseti letech na území České republiky. Identifikovali jsme odborné doporučení a guideliny, vytvořili vyhledávací strategie, díky kterým jsme prostřednictvím databází Web of Science, Medline, Scopus a Medvick vyhledali publikace. Po zavedení inkluzních kritérií jsme analyzovali zařazených 38 článků a extrahovali jsme z nich metodická data. V souboru analyzovaných studií využívalo 63 % analýz primární data pro určení přínosu a 24 % sekundární data pro určení nákladů. Většina článků využívala perspektivu plátce. Modelovací techniky využilo 42 % článků z toho 14 analýz počítalo s Markovovým modelem. Velmi málo studií zmínilo a objasnilo postup jednání s účastníky studie/zapojení pacientů, jen několik studií uvedlo diskontní sazbu a data o měně a přepočtu. Často nebyla popsána diverzita mezi podskupinami nebo uvedená práce s nejistotami v metodách. Velmi často bylo vhodně uvedené nacenění nákladů a interpretovány relevantní výsledky. Podařilo se nám prokázat zvyšující se časový trend. Hlavní limitací bylo zapojení do práce pouze jednoho recenzenta-studenta a ne výzkumného týmu expertů, jak to bývá u jiných rešerší. Dosáhli jsme strukturovaného zmapování situace klinicko-ekonomických analýz v ČR.

Clinical-economic analyzes of health technologies in the Czech Republic: Clinical-economic analyzes of health technologies are used in the HTA system to support objective decision-making by health sector authorities. The aim of this thesis was to carry out a systematic search of clinical-economic analyzes of health technologies published in the last ten years in the territory of the Czech Republic We identified expert recommendations and guidelines, created search strategies, thanks to which we searched for publications through the Web of Science, Medline, Scopus and Medvick databases. After introducing the inclusion criteria, we analyzed the included 38 articles and extracted methodological data from them. In the set of analyzed studies, 63% of analyzes used primary data to determine benefits and 24% used secondary data to determine costs. Most of the articles used the payer perspective. Modeling techniques were used by 42% of the articles, of which 14 analyses counted on the Markov model. Very few studies mentioned and clarified the study participant/patient engagement process, and only a few studies reported the discount rate and currency and conversion data. Diversity between subgroups was often not described or work with uncertainties in the methods reported. Very often, cost pricing was appropriately stated and relevant results were interpreted. We were able to demonstrate an increasing time trend. The main limitation was the involvement in the work of only one reviewer-student and not a research team of experts, as is the case with other researches. We achieved a structured mapping of the situation of clinical-economic analyzes in the Czech Republic.
