Laboratorní stanovení kalprotektinu u pacientů s Crohnovou chorobou

Abstract

Tato bakalářská práce se soustředí na laboratorní stanovení fekálního kalprotektinu u pacientů trpících Crohnovou chorobou. Ukazuje jeho výhody, užitečnost a využití v klinické praxi. Teoretická část je zaměřená na obecné informace o Crohnově chorobě její etiopatogenezi, příznaky, diagnostické postupy a formy možné terapie. Dále popisuje komplikace, které mohou tuto nemoc doprovázet. Uveden je i průběh onemocnění u specifických skupin pacientů, jako jsou například těhotné ženy nebo senioři. V praktické části se nachází kazuistiky konkrétních jedinců, na kterých je ukázáno praktické využití stanovení fekálního kalprotektinu a vztah hodnot s jejich aktuálním zdravotním stavem. Jedná se o pacienty gastroenterologického oddělení Nemocnice Písek a. s. Použité hodnoty byly změřeny v období mezi lety 2020 a 2023 v laboratoři klinické biochemie. Následuje i znázornění souvislosti mezi hodnotami fekálního kalprotektinu a jinými zánětlivými markery. Zpracovaná data ukazují, proč je stanovení fekálního kalprotektinu užitečné nejen u pacientů s Crohnovou chorobou a jak toto vyšetření přispívá k tomu, aby se jim dostalo vhodné péče. Ukazují se ovšem i určité limitace a potřeba hodnoty vhodně interpretovat.The bachelor´s thesis is focused on laboratory determination of faecal calprotectin in patients suffering from Crohn´s disease. It shows its advantages, usefulness, and utilization in clinical practice. The aim of the theoretical section is to provide the general information about Crohn´s disease its etiopathogenesis, symptoms, diagnostic approaches, and forms of possible therapy. Further the section describes the complications, which can accompany this disease. Introduced is also course of sickness in specific groups of patients, for example in pregnant women or seniors. The practical part presents the cases of concrete patients that show practical utilization of determining faecal calprotectin and relation of its values with current health condition. Furthermore, the section that showcases the representation of connection between values of calprotectin and another inflammatory markers. The thesis concerns itself with patients from the Gastroenterological department of Písek hospital. Used values were measured in the period between the years 2020 and 2023 in laboratory of Clinical biochemistry. The processed data shows why the determining of faecal calprotectin useful in patients suffering not only from Crohn´s disease, as well as how it contributes to the patients receiving suitable care. However, certain limitations arise, and the need to suitably interpret the values of the determining appear as well.
