Pilotní studie pacientských preferencí při screeningu rakoviny prsu

Abstract

V České republice patří karcinom prsu k nejčastějším nádorovým onemocněním, které postihuje tisíce žen ročně. Hlavním cílem této diplomové práce je provedení průzkumu analýzy pacientských preferencí při screeningu rakoviny prsu za využití metody Discrete Choice Experiment (DCE). V rámci analýzy současného stavu se práce zabývá studiemi, které pro zjištění preferencí žen ohledně screeningu rakoviny prsu aplikovaly metodu DCE. Nastavení metody DCE se opírá o předešlé studie zkoumající preference žen ohledně screeningu rakoviny prsu. Při vytváření DCE byly nejdříve vybrány atributy ohledně mamografického screeningu, kterými byly bolest pociťována při vyšetření, doba strávená čekáním na výsledky, spolehlivost vyšetření, doba strávená cestováním do screeningového centra a doplatek za vyšetření (mimo veřejné zdravotní pojištění) a následně u každého atributy i jejich úrovně. Tyto kombinace byly poté rozděleny do výběrových sad, mezi kterými ženy činily svá rozhodnutí. Z výsledků vyplývá, že nejdůležitějším atributem je pro ženy je bolest pociťovaná při vyšetření, dále pak doba strávená čekáním na výsledky a doplatek za vyšetření (mimo veřejné zdravotní pojištění). Naopak méně důležitým atributem byla pro ženy spolehlivost vyšetření a nejméně významný byl pro ženy čas strávený cestováním do screeningového centra.In the Czech Republic, breast cancer is one of the most common cancers that affects thousands of women every year. The main goal of this diploma thesis is to conduct a survey of the analysis of patient preferences in breast cancer screening using the Discrete Choice Experiment (DCE) method. As part of the analysis of the current state, the thesis examines studies that applied the DCE method to determine women's preferences regarding breast cancer screening. The DCE method setting is based on previous studies investigating women's preferences for breast cancer screening. When creating the DCE, firstly, the attributes related to mammography screening were selected. These attributes included the pain felt during the examination, the time spent waiting for the results, the reliability of the examination, the time spent traveling to the screening center and the out-of-pocket payment for the examination (outside of public health insurance). Secondly, for each attribute, their levels were selected. These combinations were then divided into choice scenarios from which the women made their decisions. The results show that the most important attribute for women is the pain felt during the examination, followed by the time spent waiting for the results and the out-of-pocket payment for the examination (outside of public health insurance). On the contrary, the reliability of the examination was a less important attribute for women, and the time spent traveling to the screening center was the least important for women.
