Abstract

Diplomová práce se zabývá analýzou nákladové efektivity digitálního monitoringu obstrukční spánkové apnoe pomocí chytrých nositelných zařízení v porovnání s polysomnografií v podmínkách českého zdravotnického systému. K dosažení cíle byl sestaven Markovův model s 40letým časovým horizontem. Výsledky byly hodnoceny pomocí inkrementálního poměru nákladové efektivity (ICER) při prahové hodnotě ochoty platit 1 200 000 Kč/QALY. Robustnost výsledků byla ověřena jednosměrnou deterministickou analýzou senzitivity, scénářovou analýzou a pravděpodobnostní analýzou senzitivity. V základním scénáři nevycházejí nositelné technologie jako nákladově efektivní alternativa k PSG. I když jsou chytrá nositelná zařízení levnější o 12 194 Kč, generují o 1,0 QALY méně na pacienta. Pravděpodobnostní analýza senzitivity potvrdila tento závěr v 98,2 % ze 5 000 Monte Carlo simulací. Klíčovým zjištěním práce je, že po zahrnutí reálné adherence k diagnostickému vyšetření v analýze scénářů (65 % u PSG, 95 % u wearables) se výsledek obrací, tedy nositelné technologie se stávají nákladově efektivní strategií s ICER 1 650 Kč/QALY, který se nachází hluboko pod prahem ochoty platit, díky vyššímu celkovému počtu diagnostikovaných pacientů. Výsledky naznačují, že nositelné technologie mají potenciál jako doplňkový screeningový nástroj v hybridním diagnostickém modelu, nikoliv jako přímá náhrada PSG. Práce přispívá k dosud omezenému ekonomickému hodnocení digitální diagnostiky OSA v českém prostředí.

This thesis analyses the cost-effectiveness of digital monitoring of obstructive sleep apnea using smart wearable devices compared to polysomnography within the Czech healthcare system. To achieve this objective, a Markov model with a 40-year time horizon was constructed. The results were evaluated using the incremental cost-effectiveness ratio (ICER) at a willingness-to-pay threshold of 1,200,000 CZK/QALY. The robustness of the results was verified using unidirectional deterministic sensitivity analysis, scenario analysis, and probabilistic sensitivity analysis. In the base case scenario, wearable technologies do not emerge as a cost-effective alternative to PSG. Although smart wearable devices are 12,194 CZK cheaper, they generate 1.0 fewer QALYs per patient. Probabilistic sensitivity analysis confirmed this conclusion in 98.2% of 5,000 Monte Carlo simulations. A key finding of the study is that after incorporating real-world adherence to diagnostic testing in the scenario analysis (65% for PSG, 95% for wearables), the result reverses; that is, wearable technologies become a cost-effective strategy with an ICER of 1,650 CZK/QALY, which lies well below the willingness-to-pay threshold, thanks to the higher total number of diagnosed patients. The results suggest that wearable technologies have potential as a supplementary screening tool in a hybrid diagnostic model, rather than as a direct replacement for PSG. This study contributes to the currently limited economic evaluation of digital OSA diagnostics in the Czech context.
