Akutní infarkt myokardu u pacientů s diabetes mellitus

Abstract

Bakalářská práce se zabývá problematikou akutního infarktu myokardu u pacientů s diabetes mellitus. V rámci teoretické části jsou proto popsány tyto dvě diagnózy. První část je věnována akutnímu infarktu myokardu z hlediska patofyziologie, klinického obrazu, způsobu jeho diagnostiky, vyšetření a strategie léčby. Jsou zde popsány též rizikové faktory podporující vznik aterosklerózy, tedy podkladu pro nejčastější formu ischemické choroby srdeční. Druhá část teoretické části obsahuje charakteristiku onemocnění diabetes mellitus. Je uvedena klasifikace diabetu, způsoby diagnostiky, klinický obraz, komplikace a terapie. Závěrem jsou uvedena specifika akutního infarktu myokardu u pacientů s diabetes mellitus, konkrétně patofyziologie, kardiovaskulární rizikové faktory a charakteristické rozdíly v klinickém obraze a terapii. Praktická část obsahuje čtyři případové studie. Jsou popsány a analyzovány kazuistiky diabetiků a nediabetiků s akutním infarktem myokardu s ST elevacemi a bez ST elevací. Konkrétní případy jsou zaměřeny na rozdíly v manifestaci akutního infarktu myokardu u diabetiků a nediabetiků, časové zhodnocení diagnostického postupu od vzniku obtíží a jednotlivé aspekty ošetřovatelské péče a terapie.

The bachelor paper addresses the issue of an acute myocardial infarction in patients suffering from diabetes mellitus. The theoretical section thus describes these two diagnoses. The first part focuses on an acute myocardial infarction in terms of its pathophysiology, clinical symptoms, diagnostics, examination and treatment strategy. It furthermore provides the description of risk factors supporting the onset of arteriosclerosis, the basis for the most common form of ischemic cardiac disease. The second part of the theoretical section contains description of characteristic features of the diabetes mellitus disease, along with its classification, diagnostics, clinical symptoms, complications and therapies. The concluding section addresses specifics of an acute myocardial infarction in patients with diabetes mellitus, particularly pathophysiology, cardiovascular risk factors and characteristic differences in clinical symptoms and therapy. The practical section composes of descriptions and analyses of four case studies of diabetic and non-diabetic patients suffering from an acute myocardial infarction with ST elevations and without ST elevations. These cases depict differences in manifestation of an acute myocardial infarction in diabetic and non-diabetic patients, time assessment of diagnostic progress from the onset of the difficulties, and individual aspects of nursing care and therapies.
