Analýza cesty pacienta s karcinomem slinivky břišní zdravotním systémem

Abstract

Tato diplomová práce s názvem Analýza cesty pacienta s karcinomem slinivky břišní zdravotním systémem je koncipována tak, aby analyzovala dílčí fáze, kterými pacient s diagnózou karcinomu slinivky břišní prochází v rámci zdravotního systému a zaměřuje se především na ranou fázi. Vzhledem k tomu, že tento typ karcinomu je řazen mezi nejagresivnější nádorová onemocnění s nejhorší prognózou pětiprocentního přežití, je pro pacienta zásadním faktorem čas, konkrétně doba od stanovení diagnózy až po zahájení léčby. Cílem této práce bylo provést v obecné rovině komplexní analýzu období zahrnující dobu před stanovením diagnózy a průběh jednotlivých kroků vedoucích k diagnostice. Práce se detailně věnovala analýze jednotlivých etap, kterými pacient prochází během cesty, včetně období od projevů příznaků po diagnostiku, dostupnost specializovaných vyšetření a využití služeb komplexních onkologických center. Metodologie této práce zahrnovala analýzu dat získaných z Kanceláře zdravotní pojišťovny, přičemž výsledky poskytly podrobný náhled na současný stav diagnostiky karcinomu slinivky břišní v daném zdravotnickém systému a pomohly identifikovat oblasti, které vyžadují zlepšení. Do výzkumu byli zahrnuti pacienti, u kterých byla přítomna suspektní událost a u nichž interval mezi suspekcí a zobrazovacím vyšetřením nepřesáhl 30 dní. Výsledky byly analyzovány pomocí softwaru RStudio a vyplývá z nich, že časová posloupnost mezi vybranými událostmi je v České republice srovnatelná se situací v zahraničí. Závěry a doporučení této diplomové práce mohou sloužit jako podklad pro implementaci nových strategií a postupů v diagnostice, léčbě a péči o pacienty s karcinomem slinivky břišní, a to s cílem zlepšit výsledky léčby a kvalitu života těchto pacientů.This diploma thesis, entitled Analyzing the pancreatic cancer patients journey trough the health system, is designed to analyze the sub - stages that a patient diagnosed with pancreatic cancer goes trough within the health system, focusing primarily on the early stage. Considering that this type of cancer is classified among the most aggressive cancers with the worst prognosis of the five percent survival rate, time is a crucial factor for the patient, specifically the period from diagnosis to treatment, or palliative care. The aim of this thesis was to perform a comprehensive analysis at the general level of the period including the time before diagnosis and the progression of individual steps leading to the diagnosis. The work focused in detail on analyzing the different stages of the patients journey, including areas such as the time from symptoms presentation to diagnosis, the availability of specialized examinations, and the use of services at comprehensive oncology centres. The methodology of this work included analyzing data obtained from the Health Insurance Office. The results provided detailed insight into the current state of diagnostics for pancreatic cancer in the given health system and helped identify areas requiring improvement. The research included patients who had suspected an event and for whom the interval between suspicion and imaging examination did not exceed 30 days. The result was analyzed using RStudio software and showed that the time sequence between the selected events in the Czech Republic is comparable with the situation abroad. This thesiss conclusions and recommendations may serve as a basic for implementing new strategies and procedures in the diagnostics, treatment, and care of patients with pancreatic cancer, with the aim of improving treatment outcomes and the quality of life for these patients.
