Řízená terapie hypotermií v intenzivní poresuscitační péči

Abstract

Bakalářská práce popisuje současný stav poskytování řízené terapie hypotermií v intenzivní poresuscitační péči. Byly zde porovnány přístupy a metody zdravotnických zařízení, která tuto terapii poskytují. Práce byla zaměřena na spektrum pacientů po úspěšné kardiopulmonální resuscitaci, která byla zahájena po netraumatické náhlé zástavě oběhu. Teoretická část se zabývá historickým vývojem užití řízené terapie hypotermií až po současný stav, vlivem hypotermie na organismus člověka a metodami chlazen následného ohřívání pacienta. V praktické části jsou pomocí kazuistik zmapovány způsoby a postup aplikace terapeutické hypotermie ve dvou zdravotnických zařízeních. Bylo zanalyzováno 6 kazuistik, 3 z každé nemocnice. Jedná se o kazuistiky pacientů po náhlé mimonemocniční zástavě oběhu. U některých z těchto vybraných pacientů byla terapie lékařem indikována, u některých nikoliv. Sběr dat, potřebných ke zpracování kazuistik, probíhal ve formě informací ze zdravotnické dokumentace pacientů po úspěšné kardiopulmonální resuscitaci. Na závěr, pro lepší přehlednost, byly výsledky zpracovány o tabulek.

The bachelor thesis describes the current state of providing controlled therapy of hypothermia in intensive post--resuscitation care. The approaches and methods of medical facilities that provide this therapy were compared in this thesis. The work was focused on the spectrum of patients after successful cardiopulmonary resuscitation that was started after non-traumatic sudden circulatory arrest. The theoretical part deals with the historical development of the use of controlled therapy of hypothermia up to the current state, the influence of hypothermia on the human body and methods of cooling and also subsequent heating of the patient. In the practical part, the methods and procedure of application of therapeutic hypothermia in two medical facilities are mapped by the usage of casuistry. Six case studies were analyzed, specifically three from each hospital. These are case studies of patients after sudden non-hospital circulatory arrest. In some of these selected patients therapy was indicated by a doctor, in some not. The collection of data needed for the processing of case studies was done in the form of the use of information from the medical records of patients after successful cardiopulmonary resuscitation. Finally, for better clarity, the results were processed in tables.
