Měření centrální retinální tloušťky u pacientů s diabetickým makulárním edémem

Abstract

Diplomová práce se zabývá měřením centrální retinální tloušťky (CRT) pomocí zobrazovací metody optické koherenční tomografie (OCT), u pacientů s diabetickým makulárním edémem (DME), v rámci loadingové fáze anti-VEGF terapie. Cílem diplomové práce je analýza poklesu tohoto parametru v průběhu loadingové fáze léčby a následné srovnání dvou OCT přístrojů a vlastního algoritmu z hlediska měření rozdílu CRT za toto sledované období. V průběhu studie bylo naměřeno 23 očí pacientů s DME vstupujících do loadingové fáze anti-VEGF léčby, na Oční klinice 1.LF UK a ÚVN, pomocí OCT přístrojů HRA+OCT Spectralis a Cirrus HD-OCT 5000. V rámci studie byl potvrzen klesající trend CRT a byla potvrzena hypotéza, že v průběhu loadingové fáze bude dosaženo poklesu CRT o 25 %. Na základě zjištěných výsledků byla prokázána nejen shoda měření dvou OCT přístrojů, ale také shoda měření s vlastním algoritmem DME Analyzer, formou statistické a Bland-Altmanovy analýzy.

Presented Master's Thesis is focused to measurement of central retinal thickness (CRT), using optical coherence tomography (OCT) imaging method, during the loading phase of anti-VEGF therapy, in the sample of patients with the diabetic macular edema. The aim of this Master's thesis is to analyze decreasing tendency of this parameter, during the loading phase of anti-VEGF treatment and subsequent comparison of the two OCT devices, as well as with author's algorithm in terms of measurement a difference of CRT, during this reporting period. Over the course of the study, 23 eyes of patients with DME were measured when entering the loading phase of anti-VEGF therapy, using OCT devices HRA+OCT Spectralis and Cirrus HD-OCT 5000, at The Department of Ophthalmology of 1st Faculty of Medicine of the Charles University and the Military University Hospital Prague. The presented study confirmed decreasing tendency of CRT and hypothesis, that during the loading phase of anti-VEGF treatment will be achieved 25% decrease of CRT. Based on results of the presented study was confirmed the congruence of measurements between OCT devices as well as with measurement of author's algorithm DME Analyzer based on statistical and Bland-Altman's analysis.
