Tato práce se zabývá metodami vyšetření vestibulárního systému za využití očních pohybů v důsledku vestibulookulárního reflexu. V rámci výzkumu excentricity pohybu hlavy při pulzním rotačním testu bylo navrženo zařízení pro měření pohybu labyrintů vestibulárního systému sestávající z helmice pro umístění gyro-akcelerometrických senzorů. Toto zařízení na souboru zdravých subjektů (n = 5, 25,6 ± 4,2 (SD) let) se zastoupením mužů (n = 3, 28 ± 3,6 (SD) let) a žen (n = 2, 22 ± 0 (SD) let) prokázalo rozdílné hodnoty výsledných vektorů lineárního zrychlení mezi gyro-akcelerometrickými senzory umístěnými na helmici parietálně vlevo a parietálně vpravo v průběhu pulzního rotačního testu. Tato excentricita, resp. asymetrie záškubu by mohla být, spolu s odpovědí nestimulovaných polokruhových kanálků, jedním z možných vysvětlení zbytkové odezvy vestibulookulárního reflexu v důsledku interakce otolitových orgánů s polokruhovými kanálky. Zbytková odezva byla potvrzena na souboru pacientů po neurektomii vestibulárního nervu v důsledku resekce vestibulárního schwannomu (n = 25, 51,4 ± 10,2 (SD) let) se zastoupením mužů (n = 10, 47,1 ± 8 (SD) let) a žen (n = 16, 54,1 ± 10,77 (SD) let). Zisk (Gain) vestibulookulárního reflexu byl nezávislý na kategorii tumoru. Kontrolní 66letá pacientka s bilaterální ztrátou vestibulární funkce v důsledku vestibulotoxicity gentamicinu vykazovala nižší zisk (Gain) vestibulookulárního reflexu oproti hodnocené skupině pacientů. Experiment využívající Bárányho rotační křeslo s lichoběžníkovým průběhem stimulu o maximální úhlové rychlosti 120 °/s nepotvrdil vliv excentricity rotace na časovou, amplitudovou a časově-amplitudovou variabilitu perrotačního a postrotačního horizontálního nystagmu. Variabilita byla hodnocena nelineárními metodami analýzy založenými na Hurstově exponentu, detrendované fluktuační analýze, vzorkové entropii, rekurentní kvantifikační analýze, největším Lyapunovově exponentu a nově navržené metodě vycházející z Multiscale Poincaré grafu využívající kvantifikačních parametrů elips a elipsoidů. Navržená metoda vykazuje silnou korelaci s metodami nelineární analýzy především u parametrů vycházejících z časových elips. This work deals with methods for examination of the vestibular system using eye movements induced by vestibulo-ocular reflex. In the research of head movement eccentricity during head impulse test a helmet device with gyro-accelerometer sensors for measuring the movements of the vestibular labyrinths was been proposed. This device was tested on a group of healthy subjects (n = 5, 25.6 ± 4.2 (SD) years) with a representation of males (n = 3, 28 ± 3.6 (SD) years) and females (n = 2, 22 ± 0 (SD) years) During head impulse test, the device showed different values of the resultant linear acceleration vectors between the the parietal left and right gyro-accelerometer sensors located on the helmet. This eccentricity, resp. impulse asymmetry, along with the response of unstimulated semicircular canals, could be possible explanation of the residual vestibulo-ocular reflex response due to the interaction of otolith organs with the semicircular canals. Residual response was confirmed in a group of patients after vestibular nerve neurectomy due to vestibular schwannoma resection (n = 25, 51.4 ± 10.2 (SD) years) with a representation of males (n = 10, 47.1 ± 8 (SD) years) and females (n = 16, 54.1 ± 10.77 (SD) years). Vestibulo-ocular reflex Gain was independent of tumor grade. A control 66-year-old patient with bilateral loss of vestibular function due to vestibulotoxicity of gentamicin showed a lower vestibulo-ocular reflex Gain compared to the group of patients. Bárány rotational chair test with a trapezoidal stimulus with a maximum angular velocity of 120 °/s did not confirm the effect of rotational eccentricity in the time, amplitude and time-amplitude variability of perrotational and postrotational horizontal nystagmus. Variability was evaluated by non-linear methods of analysis based on the Hurst exponent, Detrended fluctuation analysis, sample entropy, recurrence quantification analysis, largest Lyapunov exponent and a newly proposed method based on Multiscale Poincaré plot using quantitative parameters of ellipses and ellipsoids. The proposed method shows a strong correlation with the methods of nonlinear analysis, especially with the parameters based on time ellipses.
