Využití discrete event simulací v hodnocení zdravotnických technologií

Abstract

Diplomová práce se zabývá použitím discrete event simulací (DES) pro hodnocení zdravotnických technologií. V teoretické části se zabývá technikami modelování a zhodnocením výhod a nevýhod využití DES ve zdravotnictví. Také byl zpracován přehled studií, které DES pro zdravotnictví již využily. Cílem této diplomové práce bylo vytvořit DES model, který bude porovnávat dvě intervence pro léčbu karcinomu prsu: klasickou radioterapii (EBRT) a intraoperativní radioterapii (IORT). Tento model byl vytvořen v programu Witness Horizon 23. Do modelu vstupovala kohorta pacientek odpovídající výskytu karcinomu prsu v České republice. Jeho výstupy jsou průměrné náklady na léčbu a průměrné QALY pacientek, a pro každou pacientku také údaje o tom, jak dlouho strávila ve kterém stádiu onemocnění a kdy se do tohoto stadia dostala. Závěrem této práce je, že DES mají potenciál být ve zdravotnictví více využívány. Při porovnání EBRT a IORT nebylo prokázáno, že by za současných podmínek byla ekonomicky nebo alespoň klinicky výhodnější pro léčbu karcinomu prsu.

The Master's Thesis deals with use of discrete event simulations (DES) in the field of health technology assessment. In the theoretical part it describes modeling techniques and evaluates advantages and disadvantages of using DES in health care. There is also an overview of studies which have already used DES in the health care field. The aim of this Master's Thesis was to create a DES model that will compare two interventions for breast carcinoma treatment: classical radiotherapy (EBRT) and intraoperative radiotherapy (IORT). This model was made in Witness Horizon 23 software. Input of the model was a cohort of patiens corresponding with the epidemical situation in the Czech Republic. Its outputs are average costs and average QALYs per patient and also data describing time spent in every stage of the illness and when they entered each of this stages for every patient. In conclusion, there is a potential for DES to be used more often in the health care field. By comparing EBRT and IORT it was found out that under current conditions there are neither economical nor clinical benefits for using IORT as breast carcinoma treatment.
