Diferenciální diagnostika poruchy vědomí v přednemocniční neodkladné péči

Abstract

Bakalářská práce se zabývá problematikou diferenciální diagnostiky poruchy vědomí v přednemocniční neodkladné péči. Součástí teoretické části je charakteristika krátkodobých, kvantitativních a kvalitativních poruch vědomí. Dále je zde uveden popis možných příčin poruchy vědomí, způsob její diferenciální diagnostiky a možná léčba v omezených podmínkách přednemocniční neodkladné péče. Praktická část obsahuje analýzu postupu výjezdových posádek při různých poruchách vědomí a následné porovnání pracovní diagnózy určené v přednemocniční neodkladné péči s konečnou diagnózou stanovenou v cílovém zdravotnickém zařízení. Výsledkem bakalářské práce je zjištění, že ačkoliv v ani jednom z popisovaných pěti případů s poruchou vědomí nebylo úplně dodrženo vyšetření ABCDE, který může sloužit k diferenciální diagnostice poruch vědomí, tak se pracovní a konečná diagnóza shodovala ve třech případech.

This bachelor thesis deals with the problematic of differential diagnosis of a disorder of consciousness in a pre-hospital emergency care. The theoretical part covers the characteristics of short-term, quantitative and qualitative disorders of consciousness. Furthermore, it includes the list of possible causes of the disorder of consciousness, forms of differential diagnosis and possible treatment in the limited conditions of a pre-hospital emergency care. The practical part includes an analysis of emergency crews' approaches during various disorders of consciousness and a follow-up comparison of the working diagnosis determined in a pre-hospital emergency care and the final diagnosis determined in the medical facility. The conclusion of the bachelor thesis is that although the ACBDE examination, intended to serve during the differential diagnosis, was not fully observed during any of the five described cases of disorder of consciousness, the working and final diagnosis were aligned in three out of those five cases.
