V léčbě onkologických onemocnění je již standardem použití techniky s modulovanou intenzitou svazku - IMRT. Její novější variantou je Intensity Modulated Arc Therapy - IMAT (někdy označováno Volumetric Modulated Arc Therapy - VMAT), kdy se průběžně pohybují jak lamely kolimátoru, tak rameno urychlovače a celá léčba je dodána v dynamicky kratším čase. Tato technika vyžaduje jiný přístup k plánování léčby i k předléčebné verifikaci plánů měřením. V neposlední řadě může mít i jiný radiobiologický dopad na lidské tkáně. Cílem této práce je podrobně se seznámit se zmiňovanými, zejména rotačními, technikami a jejich plánováním v systému Eclipse. Dále se tato práce zabývá porovnáním nejnovějších technik (VMAT, IMAT) s těmi na pracovištích běžně využívanými (IMRT, konformní radioterapie) a to zejména s přihlédnutím k dodání požadované dávky do tumoru, chránění okolních struktur a času, potřebnému k jedné frakci. Úkolem praktické části bylo zjistit, jak se k otázkám spojeným s rotačními technikami, konkrétně verifikací a plánování, staví ta pracoviště v České republice, která je již uvedla do klinického provozu. When treating oncology patients, Intensity Modulated Radiation Therapy is a standard procedure nowadays. Its newer version is so-called Intensity Modulated Arc Therapy (IMAT, sometimes known as Volumetric Modulated Arc Therapy - VMAT), when not only the leafs of the collimator move, but also the gantry and the whole treatment session takes much less time. This technique requires a different approach to its planning and also the verification of the plans before the treatment itself. Last but not least, it can also have different radiobiological effects on human tissues. The goal of this thesis is to get acquainted with the mentioned, especially rotational, techniques and their planning in the Eclipse system. Furthermore, this paper aims to compare the newest techniques (IMAT, VMAT) with the commonly used ones (IMRT, conventional radiotherapy), specifically with respect to the delivery of the prescribed dose, the sparing of surrounding tissues and the time needed for one fraction. The task of the practical part was to determine how the hospitals in the Czech Republic which have already adopted rotational therapy as a part of their clinical treatment deal with the questions surrounding it, namely verification and planning.
