Jednou z nejčastějších poruch srdečního rytmu je fibrilace síní. K jejímu vzniku přispívá i dysfunkce autonomního nervového systému. Kardioverze je běžnou metodou pro odstranění fibrilace síní. Cílem této práce je predikce úspěšnosti kardioverze nelineární analýzou EKG signálu. Kardioverze je úspěšná, pokud se fibrilace síní nevrátí v sledovaném období, v našem případě 30 dnů od zákroku. Cílem je ověřit možnost predikovat úspěšnost kardioverze před zákrokem za pomoci nelineární analýzou. Vstupními daty jsou EKG záznamy od pacientů z FN Motol. Detekcí Pan & Tompkinsovým detektorem se získají R vlny a z nich RR intervaly. Tyto RR intervaly jsou analyzovány dvěma nelineárním analýzami a lineárně v softwaru KUBIOS. Vybrané nelineární analýzy jsou rekurentní analýza a aproximační entropie. Výsledky prokázaly, že parametry DIV, AVDL, MAXV, MAXD a ENTR z rekurentní analýzy mají signifikantní hodnotu pro predikci úspěšnosti kardioverze. Pro tuto predikci stačí běžný 12 svodový záznam EKG při klidném ležení. Aproximační entropie nebyla signifikantní pro malý počet vzorků a malé množství vstupních dat. The one of the most common heart rhythm disorders is an atrial fibrillation. Dysfunctions of autonomic nervous system also contribute to the development of atrial fibrillation. Atrial fibrillation is usually treated by the cardioversion method. The aim of this work is prediction of the successfulness of cardioversion based on nonlinear analysis of ECG signal. Cardioversion is successful if the atrial fibrillation does not return in the given period under review (in our case 30 days from medical intervention). The goal is to verify the possibility of prediction the successful cardioversion before the medical intervention. The data are from ECG signals from patients from FN Motol. The detection of R-waves and RR intervals is obtained by Pan Tompkins detector. These RR intervals are analysed by methods of non-linear analysis and are processed by KUBIOS HRV SW. Methods of non-linear analysis are recurrent quantification analysis and approximate entropy. The parameters DIV, AVDL, MAXV, MAXD and ENTR of recurrent analysis show the significant differences between two groups of patients (group with and without the recurrence of atrial fibrillation). Based on these results we can say that it is enough to use ECG Holter recording during a rest in supine position. Approximation entropy was not significant for the small number of samples and a small amount of input data.
