V této bakalářské práci byl navržen systém pro mikrovlnnou hypertermickou léčbu pokročilých nádorů v oblasti hlavy a krku. Za tímto účelem byl nejprve vytvořen 3D anatomický model hlavy pacienta segmentací MRI snímků v programu Materialise Mimics. Dále byly provedeny numerické simulace rozložení EM pole a teploty v programu Sim4Life s využitím mikrovlnných vlnovodných aplikátorů s páskovým trychtýřem pracujících na frekvenci 434 MHz. Na homogenním modelu svalové tkáně byla provedena studie vlivu šířky vodního bolusu na efektivitu léčby, která byla zhodnocena na základě parametru TC25. Nejlepších výsledků bylo dosaženo v případě nejmenší vzdálenost matice dvou aplikátorů od cíle léčby. Na homogenním modelu byly následně provedeny simulace rozložení teploty při využití matice dvou i čtyř mikrovlnných aplikátorů za účelem nastavení vhodného výkonu aplikátorů, aby došlo k pokrytí cíle léčby teplotou v rozsahu 41–45 °C. Poté byly provedeny simulace rozložení SAR na 3D anatomickém modelu hlavy pacienta, na který byly postupně namodelovány kulové nádory o třech různých rozměrech. Simulace byly provedeny pro matici dvou i matici čtyř aplikátorů. Úspěšnost léčby byla vyhodnocena na základě parametrů TC25 a THQ, přičemž nejúspěšnější byla léčba pro nejmenší poloměr cíle. THQ byl vyšší při použití matice čtyř aplikátorů, a tak by v tomto případě vznikalo na těle pacienta méně hot-spotů než při použití dvou aplikátorů. Nakonec bylo provedeno verifikační měření na agarovém fantomu za využití dvou aplikátorů. Výsledky tohoto měření byly porovnány s výsledky numerických simulací. Byly rovněž změřeny dielektrické parametry zhotoveného fantomu. In this bachelor thesis, a system for microwave hyperthermia treatment of advanced head and neck tumors was proposed. For this purpose, a 3D anatomical model of the patient's head was first created by segmenting MRI images in Materialise Mimics. Next, numerical simulations of EM field and temperature distributions were performed in Sim4Life software using microwave waveguide applicators with a tape funnel operating at 434 MHz. A study of the effect of water bolus width on treatment effectiveness was performed on a homogeneous muscle tissue model and evaluated using the TC25 parameter. The best results were obtained when the distance of the matrix of the two applicators from the treatment target was the smallest. Temperature distribution simulations were then performed on a homogeneous model using both a matrix of two and four microwave applicators in order to adjust the appropriate applicator power to cover the treatment target with a temperature range of 41-45 °C. SAR distribution simulations were then performed on a 3D anatomical model of the patient's head, on which spherical tumors of three different dimensions were successively modeled. Simulations were performed for both a matrix of two and a matrix of four applicators. Treatment success was evaluated based on TC25 and THQ parameters, with the most successful treatment for the smallest target radius. The THQ was higher when using a matrix of four applicators, and thus fewer hot spots would be created on the patient's body in this case than when using two applicators. Finally, a verification measurement was performed on an agar phantom using two applicators. The results of this measurement were compared with the results of numerical simulations. The dielectric parameters of the fabricated phantom were also measured.
