Adherence k inhalační technice u pacientů s chronickým onemocněním plic

Abstract

Základní léčbou chronické obstrukční nemoci (CHOPN) a astmatu, které patří k nejčastějším plicním onemocněním jak v České republice, tak ve světě, je tzv. inhalační terapie (používání inhalátoru s léčivem). Diplomová práce se zabývala hodnocením adherence k inhalační terapii, a to u 63 pacientů s CHOPN či astmatem. Hodnocení probíhalo ve Všeobecné fakultní nemocnici v Praze pomocí pětibodového hodnotícího nástroje „Five steps assessment“ za přímé kontroly hodnotitele a supervize lékaře. Z výsledků vyplývá, že průměrná adherence byla u pacientů 80,95 %, což je nižní výsledek než u porovnávané společnosti CEEOR (přes 90 %). Úplné adherence (100 %) dosáhlo ovšem pouze 38,08 % pacientů. Vedlejším cílem této práce bylo pokusit se nalézt vztah mezi adherencí a plicními funkcemi pacientů, tento vztah však nebyl v rámci této práce prokázán.

The basis of therapy of chronic obstructive pulmonary disease (COPD) and asthma, the two most common chronic respiratory diseases in the Czech Republic as well as worldwide, is inhalation therapy (the use of a drug containing inhaler) This master thesis assessed the adherence to inhalation therapy in 63 patients diagnosed with chronic obstructive pulmonary disease (COPD) and asthma. The evaluation was done using the “Five steps assessment” tool under the direct supervision of the evaluator and a medical doctor in the “Všeobecná fakultní” hospital in Prague. The results show that the average adherence is 80,95 %, which is less than the reported 90 % in the compared study of CEEOR. Complete adherence (100 %) was only seen in 38,08 % of patients. Secondary aim of the thesis was to investigate the relationship between adherence and patient pulmonary function, which was however not proven.
