Perimortální císařský řez v přednemocniční péči

Abstract

Cílem této práce je zmapovat resuscitační péči těhotných v období druhé poloviny těhotenství a konfrontovat techniku resuscitace s deviací dělohy a techniku resuscitace s perimortálním řezem. Práce zohledňuje nejen resuscitaci matky, ale i následnou péči o plod. Dále si práce klade za cíl prozkoumání akutních císařských řezů, zvlášť pak jejich příčinu a výsledky a léčby traumat těhotných v přednemocniční péči. Práce zkoumá problematiku perimortálního a akutního císařského řezu jak z pohledu nemocniční péče, tak péče přednemocniční a hledá jejich hlavní rozdíly. Práci je třeba vnímat jako propojení porodnictví - traumatologie - urgentní medicíny - záchranné služby. Práce má za cíl přinést povědomí o této málo diskutované problematice na záchrannou službu a prozkoumat, zda se jejich postupy a vybavení slučují s aktuálními poznatky v medicíně s touto problematikou spojenou. Hlavní přínos práce spočívá v analýze kazuistických případů, vlastního průzkumu a interpretaci výsledků, jejichž závěry mohou přinést nové resuscitační a ošetřovací postupy, prosadit odborné kurzy této problematiky ve zdravotnické záchranné službě a pomoci tak zachránit nejdražší životy žen a dětí.

The aim of this work is to map the resuscitation care of pregnant women during the second half of pregnancy and to confront the technique of resuscitation with deviation of the uterus and the technique of resuscitation with perimortal section. The thesis takes into account not only the resuscitation of a mother but also the subsequent care of the fetus. Furthermore, the aim of the thesis is to investigate acute caesarean sections, especially their cause and results and the treatment of traumas in pre-hospital care. The work examines the issue of perimortal and acute caesarean section both from the perspective of hospital and pre-hospital care and looks for their main differences. This work should be seen as interconnection of obstetrics, traumatology, emergency medicine and emergency services. The aim of the thesis is to raise awareness of this rarely discussed issue of the emergency medical service and to investigate whether their procedures and equipment match with current medical knowledge related to this issue. The main benefit of this work is the analysis of case studies, own research and interpretation of results, which can bring new resuscitation and nursing guidelines, enforce professional courses on this issue in a medical rescue service and help save the most valuable lives of women and children.
