Možnosti fyzioterapeutických přístupů u cévních mozkových příhod se zaměřením na zrcadlovou terapii

Abstract

Cévní mozkové příhody jsou aktuální problematikou, jelikož patří mezi nejčastější příčiny úmrtí a jejich incidence stoupá. Pro přeživší je rehabilitace důležitou součástí léčby. Předmětem bakalářské práce je zhodnotit efektivitu zrcadlové terapie v rehabilitaci ruky u pacientů v subakutním stádiu cévní mozkové příhody. V teoretické části bakalářské práce je pojednáno o etiopatogenezi, klinickém obraze, diagnostice a terapii cévních mozkových příhod. Kapitola metodologie se zabývá rehabilitací a testováním pacientů po cévních mozkových příhodách a fyzioterapeutickými přístupy použitelnými u ictů včetně zrcadlové terapie. Ve speciální části je souhrn anamnestických údajů a údajů o soběstačnosti, schopnosti úchopů, míře spasticity a spazmů získaných na začátku a na konci terapie u dvou skupin probandů. První skupina pacientů je kontrolní, rehabilitace zahrnovala fyzioterapeutické postupy popsané v metodologii, a ve druhé skupině jsou nemocní, u kterých byla nad rámec klasické cvičební jednotky aplikována zrcadlová terapie. Ve výsledcích jsou zhodnocena zjištěná data ze vstupních a výstupních vyšetření u jednotlivých pacientů a porovnány výsledky terapie mezi skupinami probandů. V diskuzi jsou tyto údaje porovnány se závěry vědeckých studií. Klíčová slova: rehabilitace, cévní mozková příhoda, hemiparéza, zrcadlová terapie.

Cerebrovascular accidents are the current issues because they belong among the most frequent causes of death and their incidence is increasing. For those who survive, the rehabilitation is an important part of treatment. The subject of this thesis is to evaluate the effectiveness of mirror therapy in the rehabilitation of the hand of patients with subacute phase of strokes. The theoretical part of the thesis deals with the etiopathogenesis, clinical picture, diagnosis and treatment of strokes. Methodology chapter deals with the rehabilitation and testing of patients after stroke and physiotherapy approaches applicable after stroke, including mirror therapy. The special part is a summary of the anamnestic data, data about self sufficiency, grip skills, level of spasticity and spasms obtained at the beginning and end of therapy in two groups of probands. The first group of patients is comparative unit, in rehabilitation of the group were included physiotherapy procedures described in the methodology, and in the second group are patients which was, in addition of classical exercise unit, applied mirror therapy. In the results are evaluated the data from the input and output tests of the individual patients and compared between the groups of probands. These data are in the discussion compared with the findings of scientific studies. Key words: rehabilitation, stroke, hemiparesis, mirror therapy.
