Analýza diagnostické shody u bederní páteře mezi MDT (McKenzie) terapeuty dle typu a způsobu vzdělání

Abstract

Diplomová práce se zabývá analýzou diagnostické shody u bederní páteře mezi třemi různými skupinami terapeutů Mechanické Diagnostiky a Terapie dle Robina McKenzie (dále MDT terapeuti). Skupiny tvoří: 1) studenti magisterského studia po dvou semestrech povinné výuky konceptu Mechanické Diagnostiky a Terapie dle Robina McKenzie (dále již MDT konceptu). Průměrná časová dotace výuky byla 3 h týdně. Tito studenti zároveň absolvovali třídenní kurz MDT část A, zaměřený na bederní páteř, který byl doplněn o on-line přípravu před samotným kurzem. 2) absolventi 3denního MDT A kurzu doplněného rovněž o on-line přípravu před kurzem s různě dlouhou dobou praxe a různou historií jiných kurzů a vzdělávání v různých konceptech mimo MDT přístup. 3) Třetí skupinu tvoří certifikovaní MDT terapeuti z praxe, kteří absolvovali kompletní kurzy A-E a certifikační zkoušku. Teoretická část se zaměřuje na problematiku bolesti dolní části zad včetně epidemiologie a rizikových faktorů. Dále je zde popsána anatomie a biomechanika bederní páteře a přiblížení McKenzie konceptu. Metodika popisuje postup sběru dat a charakteristiku zkoumané skupiny. Kromě toho obsahuje podrobný popis McKenzie terapeutického spisu. V kapitole výsledky jsou uvedená veškerá data o zkoumaných skupinách a následně prezentace získaných výsledků. Výsledky ukázaly, že certifikovaní MDT terapeuti dosahovali nejvyšší úspěšnosti i největší vnitroskupinové shody, což potvrzuje význam vzdělání a praktické zkušenosti s metodou.

The thesis deals with the analysis of diagnostic agreement in the lumbar spine between three different groups of Robin McKenzie Mechanical Diagnosis and Therapy therapists (MDT therapists). The groups are: 1) Master's students after two semesters of compulsory training in the concept of Mechanical Diagnosis and Therapy according to Robin McKenzie (hereafter MDT concept). The average teaching time was 3 hours per week. These students also completed a three-day MDT course - Part A, focusing on the lumbar spine, which was supplemented by online preparation prior to the course itself. 2) Graduates of the 3-day MDT A course, also supplemented with online preparation prior to the course, with varying lengths of experience and varying histories of other courses and training in different concepts outside of the MDT approach. 3) The third group consists of certified MDT practitioners who have completed the full A-E courses and certification exam. The theoretical portion focuses on low back pain including epidemiology and risk factors. It also describes the anatomy and biomechanics of the lumbar spine and introduces the McKenzie concept. The methodology describes the data collection procedure and characteristics of the study group. In addition, it includes a detailed description of the McKenzie therapeutic file. In the results section, all data on the groups studied are presented, followed by a presentation of the results obtained. The results showed that certified MDT therapists had the highest success rates and the highest intragroup agreement, confirming the importance of education and practical experience with the method.
