Fyzioterapeutická péče o pacienta po amputaci dolní končetiny

Abstract

Bakalářská práce je zaměřena na fyzioterapeutickou péči o pacienta po amputaci dolní končetiny. V mém případě obě pacientky podstoupily amputaci ve stehenní oblasti, proto je práce směřována spíše tímto směrem. Práce je rozdělena na tři hlavní části: obecnou část, metodologii a speciální část. Obecná část začíná stručnou anatomií dolní končetiny, ale především obsahuje informace o amputacích, jejich příčiny, druhy, komplikace, také je zde popsán vlastní operační výkon. Velká část je věnována problematice amputačního pahýlu, obzvláště správné péči o něj. Dále se věnuji oblasti protetiky, která zahrnuje stavbu a typy protéz nebo materiály na jejich výrobu. Obsáhlejší částí je kapitola o komplexní rehabilitační péči, kde je zařazena rehabilitace před amputačním zákrokem i po něm, včetně fyzikální terapie, ergoterapie, nácviku chůze i nácviku pádů. Kapitola metodologie zahrnuje sběr dat od pacientů, popis pracoviště, ve kterém byla práce zpracována, a jsou zde vysvětleny vyšetřovací i terapeutické metody použité ve speciální části. Speciální část je zpracována formou dvou kazuistik a zabývá se fyzioterapeutickou péčí o konkrétního pacienta. Součástí je anamnéza, vstupní kineziologický rozbor, na jehož základě je navržen rehabilitační plán, průběh terapie a výstupní kineziologický rozbor. V závěru jsou porovnány vstupní hodnoty s výstupními, zhodnocen účinek terapie a splnění daných cílů.The bachelor thesis is focused on physiotherapy care of a patient after lower limb amputation. In my case both patients underwent amputation in femoral region, therefore the work is more geared to that direction. The work is divided into three main parts: general part, methodology and special part. General part begins with a brief anatomy of the lower extremity, but contains especially some items of information about amputations, their causes, types, complications, there is also described operating intervention. A large part is dedicated to problems of amputation stump, especially to proper care of it. Further I deal with prostheticsa area which includes construction and types of prosthesis, or materials for their production. The chapter of comprehensive rehabilitation care forms a considerable part and it includes rehabilitation before and after amputation intervention, including physical therapy, ergotherapy, gait training and fall training. The chapter of methodology includes data collected from patients, description of the workplace where the work was processed and there are explained investigation and therapeutic methods used in the special part. The special part is processed in the form of two case studies and deals with physiotherapy care of a particular patient. Its parts are anamnesis, input of kinesiology analysis on the basis of which a rehabilitation plan is designed, process of therapy and output kinesiology analysis. In the end the input values are compared with the output values, therapy effect and attainment of the targets are evaluated.
